ALS Ontario VOLUNTEER AWARDS 2011

Do you know an outstanding volunteer who has made a difference?

Each year ALS Ontario honours individuals who have made significant contributions to the ALS
community and to ALS Ontario. Now is the time to nominate those you think have made
valuable contributions to let them know their efforts are appreciated.

Why?
Volunteers are the foundation on which we build support within the ALS community, so why
not celebrate their achievements and nominate them for an award?

Who?
Volunteers are people who dedicate their free time to ALS Ontario in a multitude of
capacities from peer support and administration to fundraising – each individual’s contribution
is priceless.

What?
All awards will be presented at this year’s Volunteer Awards ceremony entitled Festivale! A
Celebration of Giving on Saturday, October 15, 2011. Nominations for the awards may be made
by any person and will be reviewed by a Selection Committee. Current members of the Board
of Directors are not eligible to receive the awards. Nominees must be residents of Ontario.
The nomination guidelines below will direct those wishing to submit nominations which must
be received by August 31st, 2011.

NOMINATION GUIDELINES
To nominate a volunteer please complete the following and forward to Kelly Entwistle by August
31st, 2011.
Volunteer Award Nomination Form, including their achievements and contributions.
(bullet point is accepted)
 One Supporting letter written by an individual who has knowledge of the nominee and
his/her contribution to the ALS Ontario Cause
 Posthumous nominations are allowed.

Excellence in Fundraising Award - Evelyn Wilson of Carrying Place
Excellence in Support Services Award - Barb Scanlan of Thunder Bay
Excellence in Public Awareness Award - Dr. Jeff Sutherland of Georgetown, Tim Robertson of Oakville and Brian Dyck of Ottawa.

Congratulations to the winners of ALS Ontario's two new awards to recognize an individual under the age of 26 who has made a significant contribution to the ALS community (Rising Star Award) and to an individual who has made an enormous impact through Walk for ALS achievements (Walk N Roll Award).
